Help required in Filling out FBI Forn

Hi All,
I did my finger Printing in local Police department in houston. I have some queries about the Information I need to fill in on the FBI Fingerprinting form.

In the form there are places where I have to fill in
1) FBI Number
2) Armed Force Number MNU
3) Miscellaneous Number MNU
4) SSN
5) Your Number OCA
6)ORI

I dont have numbers for FBI, Armed Forces, OCA, Miscellaneous.What do I do?

My wife does not have a SSN but she has a Tax Identification Number. Where should I put that.
Also what is ORI(A big block below Last Name and First Name). What do I have to fill in ORI Block.
