Got a problem requesting Caips Notes

I sent a request of the caips notes to the citizenship and canadian immigration office in Ottawa. This is the form that I sent
http://www.cic.gc.ca/english/pdf/kit...s/IMM5563B.pdf
I got a letter from someone in the office with the following statement
"we need your written consent to process the request. To be valid, a consent must be signed, dated, and indicate to whom authority is given with the printed name of the person giving consent"
Is there another form for this or just a simple letter will do it

Hi MadHatters
I would like to get some advice from you,
I'm going to order CAIPS as you did, I mean trought my friend in CA.
I have two essential forms to do this, but I'm not sure:
1. How and where should I send money (5 CAD ?) ?
2. Where shoul I send those two forms ?
3. How long had you waited ?

Basically I sent 7USD to account for mailing expenses (2$) and the 5CAD that you have to pay for the caips. So, basically you fill the forms out, and mail them to your friend along with the USD. Your friend will mail the forms and a Canadian check to the office in Toronto. On my previous threads you can find the forms and the address. It will take 1 month to get back to your friend and then he/she will mail it to you. My friend faxed me first. I do not remember the address right now but do a search on the cic web site. On my previous threads there is a link from someone else that takes you to the address where yout friend has to mail it.
