Default Clarifications on IMM1249E (application to change conditions/extend stay)

Hi, all. I was wondering if you could answer a couple quick questions
for me.

I'm a US citizen, my same-sex partner is Canadian. We've been living
together in Nova Scotia for over 4 years. I am currently on a student
visa which expires June 30. As a condition of graduation from my
program, I have to complete a work experience component, so I am
submitting the form in order to change my conditions to a work visa
instead. I have a formal offer from a company here in town.

I'm confused on a couple of things though:

1. What fee do I pay? In the application kit, it outlines fees for
both the extension of temporary resident status (which I'm not--the
work experience will end prior to my student visa expiring) and
restoration of status (which I'm also not--that sounds more like
geting your status restored if you let it expire accidentally). Also,
would someone locally like a bank or post office have these receipt
forms so I didn't have to wait another couple weeks? (the company
wants me to start basically as soon as I can get a proper visa)

2. Under the "Family Members" section, do I put my partner's details?
Or do they mean only document spouse/common-law partners who are
non-Canadian residents who are with you in Canada?

3. If I am supposed to submit my partner's details on the form, I'm
assuming I also have to send off a completed IMM5409E (declaration of
common-law union). Where can I find a "Commissioner of Oaths", and
what date do I put as the common-law relationship having started? The
1-year anniversary from when we moved in together, the date when we
registered our domestic partnership (about 2 years later), or..?
