Children Immunization Record. Please HELP!

Children Immunization Record. Please HELP!
We are on work visa in USA and now planing to land canada next week. Most of my kids imunization is from asia. I have two children under 16 years old. I need to fill up Immunization
record form and certify by a qualified health professional. At the end
of form, it ask information about health professional such as:
"SIGNATURE OF QUALIFIED HEALTH PROFESSIONAL", "DATE","NAME","ADDRESS"
and "PROFESSIONAL DESIGNATION".
There is a fields that confuse me. I don't know what to fill in
"PROFESSIONAL DESIGNATION" field.

I received this form with our passports and it says that fill it get it certified before coming to canada. It just confused me. Will they ask for it at the landing time. I will get a immunization record letter and proof from the current school , I hope it works as I dont have enough time left to do all this as I am landing next week. If anybody has landed with kids , please clarify on this.
