Change the application to a different stream
Is there any specific procedure to change an application from one stream to another. Let's say Skilled Worker application to Family class or Provincial Nomination?
Is it necessary to pay an extra fee?

Nope. You must withdraw your application and re-apply in different class.

Same thing - rules now say that once application is received the category cannot be changed, even within the same class.

There is no regulatory authority (provision) allowing change of categories (or even principal applicants for that matter) once application is made. You'll find confirmation of it in relevant chapters of Immigration Manual (for example read sections 5.9 and 5.10 of OP8).
