Hi experts,

In application kit for family class, under Appendix C - Checklist, in the
requirements for Immigrant Application Form (item 1), there is a sentence:-

"If you want us to deal with a Canadian representative on your behalf, be sure you
have signed a document allowing us to release information, on the back of the
application form and on a separate document. Your authorisation must identify a
person who may receive information on your behalf."

The question is: Is there any standard format of this kind of authorisation letter
document that is available in the Canadian immigration website? If not, can any
immigration professional give me a sample of this document?
