Post Application fees

I filled in the simplified application and paid 1400 dollars at the time. I have recieved the 90 day letter. Do i submit another 1400 dollars for the application processing as well as Permanent residence fee,(if i choose to at this time, PR), or have i already paid. I have the 90 day letter but dont want to delay by not submitting correct paper work. I am confused, anyone help!?

You can safely wait until you are requested to take the immigration medical.
