Additional Family Information Form 5406
Out of interest, why do I need to list my parents information ?
We have been estranged for over 6 years, and I can only assume that their contact information hasn't changed.......

So if you decide to sponsor them down the road, the information is there for cross checking. Don't forget the forms aren't designed for individuals, they are for everyone.

I know the forms aren't designed for individuals....was just concerned as I have no idea of the accuracy of the information I could provide. Not even certain of their year of birth.....but if that's all the info is going to be used for, I can't imagine it would matter too much
