Default UKVI call center - questions on how do they know

When you call or send an email to the UKVI do the good folks who respond to your questions actually have access to your application.?

if they don’t have access how can they tell you or send you an email saying it’s in the hands of a case worker ? ( some threads indicate this is the response they receive )

When you request to escalate what do the good folks in the call center actually do to escalate the case ?
