Residency questionnaire- social ties

Hi,
I received a residency questionnaire after applying for citizenship. My common-law partner is a canadian citizenship since birth. For question 11 where it asks 'since your arrival date in canada, for each family member of your immediate family provide legal citizenship/immigration status in the country they reside.' For the 'date status obtained in canada' im assuming i'll write his DOB?
And then i get to section 'other.' So i have to give health records detailing the dates of which I received medical services since I've been living here? I don't see how that can prove I was living in Canada in certain years, I arrived in Canada in 2009, have been here ever since and didn't visit the doctor until approximately 18 months later! So really that doesn't prove your residency.I guess ill just get my file which will list the 6 times i've been to the doctors over here....
Anyways my main question! Its asking me to describe the nature and extent of my social ties. Examples they give are being an active member in the community/religious organisations/volunteer groups.
I don't volunteer, I'm not religious. I do have friends though! However i'm not sure how to prove that? Send in photos of us socialising?!!! How did others prove their social ties if you are like me i.e. employed in a 9-5 job and social on weekends/evenings etc. I do own a house here with my boyfriend, we have a dog, very close to his family who live nearby. Should I get letters from family and friends?

I would say you probably need to get references from your friends, stating how they know you, how long they've known you, etc
