hi, my address and last name has changed..so i need to send my passport and PR card off for name change..also my address has changed do i need to inform both of the address change as i noticed neither have a address on them...also can anyone give me the links for both to change my name etc

No, you don't report addresses changes, but of course you have to give the new address in the application when you apply for the PR card and the passport else they won't be delivered.
