Complete the application form.
The service is by appointment only - phone your nearest British embassy, consulate or high commission and select the ‘consular services’ option.
Take your passport, application form (and 1 other form of photo identification if you have it) to your appointment - parents can attend on behalf of children, and husbands and wives can attend on behalf of each other.

I did this at the end of June for my passport that was 5 months expired and my daughter whose passport was 4 months expired. I made the appointment at the phone to visit the Toronto office at 777 Bay Street. We handed in the forms and passports (no other ID was asked for but we had taken PR cards just in case).
It was all done in less than 10 minutes. You are also handed a letter that you need to show with the passport when you travel. I am aware that the U.S doesn't accept the extension but that's fine by me. I shall send the passports for renewal proper early next year.

They told me I had to wait until yesterday to make the appointment (so, 7 months before expiry).
It's free - except the damned flight to Vancouver of course. Thank you Airmiles!
I would have sent it for renewal but could not risk any delay as the trip I need to make is of paramount importance.
