Passeport Requis

Hello,
I have applied for PR from Paris, France. I have finished my medicals, paid ROLF and the ECAS status is 'Decision Made'. Now I have received a document in French called 'Passeport Requis' There is a date on the start of the document as 27/-6/2003.
I need some informationon this document what is that I need to do with it.
There is a form with details and a photograph specification sheet. The form has two annexures.
Could anyone help me in this? Is there a englich version of this , if yes how can I get it?

Could you give me your timeline from the date you sent your application.
What's your country of origin?
I am going to submit my file in Paris in few weeks,

Looks like they want your passport to put the visa in? that what's means
"passeport requis"..for the date I do not know, is it the validity limit of
your medical, or the one of your passport? The earliest of these two
validity dates give you the validity date of your permanent resident visa.
As per the Canadian Embassy website in Paris, if you do not use your visa
within its validity and still wish to immigrate to Canada, you will have to
submit a new application and pay new processing fee.

My timeline is as below:
Applied - dec 2000
File No. received - Jan 2001
Interview- Waived
Medicals- Jul 2002
ROLF- Aug 2002
Passport Requis- Sep2002
Country of Origin- India/ UAE
