Default Maintaining Residency

I read recently that permanent residents were maintaining their residency by having an address, bank account etc in Canada, but then leaving the country.

But how come the government doesnt know they have left the country?
And why doesnt the governement know when they re-enter? Do the permanent residents simply lie? Surely the government should be able to work it out?

Or do residents simply find a way to enter and leave the country without going through immigration? Is Canada like the UK - the government just doesnt have a clue who is entering and leaving?
