LMO Help Please

Hi all,
I have only just become a member but I've been using these forums since the start of the year, they have been an invaluable source of information so thank you everyone for posting.
Here is some background of where we are in the process. My partner and I submitted our PR application at the start of the year. Shortly after we found out about BUNAC's working visa's and both applied. We were both lucky enough to get one an arrived in Calgary at the end of July to start our year here, loving it so far! I now have a job working for IT services company in Calgary who are happy to apply for an LMO to aid our PR application and so that I can continue to work for them. We are working through the form together as this is the first time they have gone through the process as well. We are planning to submit the LMO online but we're completing the paper copy first so we will have all the info to hand. I now have a list of questions about completing the LMO application which we are unsure how to answer. Most of which are silly little things but I want to make sure there are as few stumbling points as possible. I would greatly appreciate all your input.
Q20. Number of foreign workers employed currently? - They have no other foreign workers other than myself, should they enter 0 or 1 in this box?
Q34. Is the job temporary with intent to permanent? - They have given me a permanent job straight away, should this question be marked yes or no? I guess it should be yes as really I can only be employed permanently once I am a PR?
Q36. Expected duration of employment, days. weeks, months, years? - Is it ok just to put a tick in the years box?
Q45. Asks for your salary per hour, per day, per month and per year. - Do you have to complete all these boxes or is just the annual one ok?
Q52. Asks for the potential benefits of employing a foreign worker. We ticked the 'filling a labour shortage' box. It then asks for details. - Should we add here that they are still advertising to recruit another member of staff?
Q53. Asks if they plan to train for the position. They do not plan to do any specific training so we have ticked no?
There is a declaration page at the end to be signed by a third party. Is it ok for another employee of the company to sign here?
Should we include a copy of my job offer letter/contract with the LMO application form?

I have now spoken to Warren. He was very helpful and has cleared up my questions.
I would be very interested to hear other peoples experiences and views regarding LMO's, particularly in Alberta.

I don't know if it depends on what field you are in, but my husband had to get a new one and it came the next day, however he is in the building trade so that may be why.
