Contact your UK pension scheme administrators and ask them about the documentation they will need to effect a transfer of your UK scheme benefits to an Australian superannuation fund.
I recommend giving them a telephone call.
If the process of arranging a transfer looks too difficult after you have had that conversation and seen the forms/documents needed consider engaging a professional advisor to assist you.

I will, hopefully, be transferring my NHS pension into my Industry Superfund.

Of the 5 pension providers we contacted they were the least helpfull, and only seemed to want to communicate by letter, where'as the others gave figures/details over the phone.
The end result was basically get a transfer value from the UK pension company's print a form from my existing Aus Super fund, fill in the details and send it back to them, they supposedly then contact the pension companies and do the rest. Still early days as only about a month in.

My wife is in the process of transfering over her NHS pension, unfortunately the pension is in her maiden name so thats added confusion to the process..

I wasn't envisaging that sort of fee !!
