New Design DIMIA Forms Effective 01/11/2002
Details of new forms to be used from 01/11/2002 are now on the DIMIA site here:

You can download the new forms as pdf's from the DIMIA site.
Old style forms will almost certainly be acceptable for the foreseeable future - as George Lombard has said on an earlier post the requirement of a valid visa application is to submit a form xyz and it is still a form xyz even if the Design Date changes. However, if you have the choice try to submit on the new design.
