I want to do my medicals in advance. Can somebody guide me what all forms and things I have to carry with me when I go for Medicals so that i dont miss out anything. We are 3 (myself,wife and 2.2 yr old daughter).

Hello - I have not got the website link, but if you go onto the gov web site they are on there, a main form and a radiology form - however , when you book your meds our doctor told us they do have copy of the forms, it maybe worth an ask - if you dont have download facility!

I had to take vacination records of my kids.So maybe a good idea to take the baby book with you.

do not forget your meds are only valid for 1 year also all 3 will need full meds and a chest x ray for anyone over 11.depending on your visa sub class ref;you would usually require form 26 for your daughter and 160 for you and your wife 1 each.your panel doc will need these to send off to the LCU in sydney also dont forget YOUR PASSPORTS.
