Help with police check form

I have now had my medicals and police check requested. I have found a form on the South Yorkshire Police website which is a SA1 form is this correct and the cost is Â£10. Thanks for your help.

Not sure if the form is SA1...but it does cost Â£10...pop in there and get one...

Not sure about the SA, but if SA stands for Subject Access, then you are on the right track. The one you want is called a Subject Access Application Form.
