Forms for medicals and X-rays

Forms for medicals and X-rays
Have just been on DIMIA website to find the above forms and it gives 2 types of 160 form for the x-rays. One is for use only in Oz, but the other states it is only for applicants applying for a medical practitioner visa. Is this the right one? Maybe I am being thick!!!!!!!

Use forms 26 & 160 from this link:

Only use if you want to "Front Load" as your Case Officer will send them (with some details completed) to you.

Guess it's best to be thorough, if there's anything wrong it can be seen to straight away.
