Hello my name is Bradley Lodge, im currently getting all the required forms together for my life partner visa. I have everything except the BI-811 medical certificate signed. I took it to my GP and he wouldnt sign it as he said it wasnt a very clear form for the procedures needed, so i took it to a private GP who said its best to be done by a doctor who knows me. What do i say to the doctor for getting this form signed, do i just say it is a general check up? im really really stuck on this, he has said that usually you need a full medical report which could take months! has anyone else had this issue or knows how to solve it? any information is highly appreciated!

I feel for you my friend. I remember when I had to fill this as one of many forms out when I needed to emigrate to South Africa to be with my wife although luckily for me I knew my doctor very well and explained clearly my reasons why I needed this form completed and didn't even have to have a full medical. I think the doctors you have seen are just being unreasonable. Nothing particularly complicated to understand about the form it's all pretty straightforward. I'd try another doctor if I were you and if they insist on a full medical then bite the bullet and get it done after all it may seem an extreme way to get a form filled but if that is the only way then so be it.
Incidentally you will have to get chest X-ray done and payment will be required. Not all hospitals will do this though especially as it is for visa requirements. I had mine done in Hayes back in 2010 and I believe it cost me around £70 then.
Good luck

I looked at this form and I think it just means to the best of the doctor's professional knowledge, based on your medical history - which will include any record of mental disorders - and a physical examination. I don't believe the SA goverment expect him to carry out tests for all the diseases cited, plus any other "contagious condition", and refer you to a specialist to check for "all psychoses" etc. That would be absurd.
The form doesn't state that you must consult your usual doctor, or that he has to have known you for any particular period, so find another doctor and ask him to request your records.
