Generally I dislike using 3rd parties but by the sounds of it, in this case they will be useful then.

So pre-existing should be covered from day 1, but the premiums will be higher. How far in advance should I be setting this up properly? Many "instant quotes" only allow up to two months in advance. We have a vague "late November / early December" aim which I appreciate is a long time from now but better to be prepared.

Any opinions on whether I should just pay up for the family despite them being outside the country for a couple of months?

I think now would be a good time to start a relationship with someone. We had such a great broker for our family business, but she's retired now (and in the wrong state for you).

I would then ask the broker if there is an advantage, or a way around, paying for your family while they are not in the US.

Consider this like hiring any other expert. Surely, with your 'O' capabilities you can appreciate specialists.

Got a trade body, chamber of commerce or anything like that you could join? If you can, you can then often get a group rate through them, usually expensive still but gets you off the pre-existing problem etc.

The NHS credible coverage means you've got 63 days to find insurance before insurance can use pre-existing as a barrier to coverage. That's not to say they won't price you out of the market though, just that they'll offer you coverage.
