In other words, you don't seriously think that the credit card company is going to pick up the entire tab, no questions asked, when someone with high blood pressure, heart disease, poor circulation, diabetes, and a history of seizures ends up in intensive care while relying on the credit card insurance coverage, do you?

They operate on different T&C to the UK which is what the OP's parents would be looking at.

My french DEBIT card (from Barclays Fr) paid all my son's Andorran hospital bills because we used it to pay for the lift tickets. Completely forgot that Andorra wasn't in the EU so EHIC was no use.

That was pretty much my point - that if you have medical conditions that have the potential to turn into expensive medical treatment and/or require medevac repatriation you should be not be relying on default coverage provided by a credit card.

Er, you did. Being elderly is an additional risk condition, that likely excludes them from default credit card coverage, especially if they have any history of age-related medical treatment or medication.

Elderly family members with significant pre-existing conditions have used both Avanti and World First to travel to USA. Both are excellent, good coverage and sensible questions to gauge the condition.

Premiums were relatively affordable.

Thanks for all of the guidance and info. I eventually found some suitable insurance using the following. (Not meant as a plug, just as a reference to help others.)
The policy I settled upon had the usual types of travel coverage; trip interruption, lost baggage etc etc with typical type of financial compensation.
The medical coverage uses an American PPO (the majority of my local doctors and hospitals are in its network).
Coverage max is $300k, reduced to $100k for pre-existing. With a repatriation coverage of $1m. A deductible of $250 applies with no co-pays.
Total cost was $430 for the two of them.

Worth keeping in mind: Many insurance companies require you to notify them if you are prescribed new meds within 3 mths of travel. The doctor changed my MIL’s BP meds, when I told the insurance company they said they could no longer insure her and cancelled the policy.

I have heard of Insubuy, they seem to focus mainly on the Indian market, did look to see who the Underwriters are and they use a variety none of which I am familiar with. You are of course buying a US product.
