I just know when we went union at the airline I was at, we had to call a number and vote either yes or no, yes needed a majority to bring the union in.
If someone did not call, they were not counted either way.
Clearly things have changed since then.

There's never been a referendum on independence for Scotland. If you mean the proposed referendum of Nov 2010, that would have used a simple majority of votes cast. Or perhaps you mean the devolution referendum of '79, which used a threshold of 40% of the electorate voting in favour? Or the '97 referendum on devolution? Not sure what the rules for that were but again 50% of the electorate didn't vote in favour of either question.
I have a vague recollection that one or more of the Quebec independence votes might have had a plurality of the electorate requirement.
