A state is a country, and the United States were conceived as a union of countries. Early on after the declaration of independence, it was common to refer to the United States in the plural, "the United States are ...", reflecting the idea that they were individual states that allied together for some matters, but were essentially quite separate and largely self-governing..

even if he can implement a ban based on religion will it not need approval from the court which might take as long as the visa bulletin says it takes for sponsorship?

Unlikely that he will be elected, unlikely that he could get unconstitutional laws passed. But we would also be foolish to not take him at his word.
