Can someone please tell me what forms need to be filled out and submitted to the US Embassy in London for a child accompanying the parent to the USA on a Fiancee Visa as part of packet 3. Also what forms do i fill out and sign and return to the US Embassy for the provisonal file..is it the whole of packet 3 including the sworn statement and affidavit of Support and copies or originals of documents asked for such as birth certs and police record etc? Is it also best to send these documents and forms via courier?
