I printed the "card" off the net from the NYS Voting registry

In bold it tells you if you are not a US Citizen do not complete this form.

Also asks when and where you last voted and name used.

You must also swear and/or affirm that you are a US Citizens. If you falsify the information, the fine is up to $5,000 and/or jail for up to four years.
