More questions ;)
On form DS-156 question 28. asks: "Have you ever been issued a US visa?". I am a UK citizen and have not personally applied but I suppose I have been issued one under the visa waiver programme - it this correct? If I have then what do I put under "When?" and "Where?" and "What type of Visa?"

If it was me, I would put NO and in a blank put in quotes "visa waiver only".

When I was confronted with the same question, I thought the same as you. However, I answered the question with a "no" because using the visa waiver means you did not in fact even have to use/apply for a visa to enter the States.
I think the key word is "waiver". I hope this makes sense.

The answer would be "no". You have never been issued a visa.
