I am going to guess that person who worked for 5 years at the Supermarket did so to obtain Health Insurance through the Employer, probably not practical in your case.

You do not have to but through the Exchange theoretically, not sure who else would be interested in covering them before they become eligible to buy Medicare.

And of course there is a lot that is not covered by an OCare policy, so they would need a slush fund to pay for that. Or you need deep pockets.
