please could someone advise which form I need to enter as a L-2 spouse.

The same one that you need for all non-immigrant visas: DS-156 and DS-157. Of course, you will also need supporting documentation, such as a copy of your spouses' L-1 petition and a copy of your spouse's I-94 (if he is in the US) and of each page of his passport. Don't forget your marriage certificate. |
