L-1a visa and spouse L-2
 
My wife is entering the US on a L-1a visa and as a spouse I will enter on a L-2. Do we need to apply for these together or do I need to wait for her visa to come through to get any numbers / codes from it? I have seen her form and it doesn't have any space on it for a spouse so I guess I need a seperate application ?

You apply for your L-2 at the consulate after your spouse receives L-1
petition approval.

You don't need to apply together, but if you apply separately you will need to submit
evidence of that your wife has been issued an L-1A visa, including the notice of
approval of her petition. If she is in the U.S. you will need to submit evidence that
she is maintaining status there (e.g. most recent payroll stub). This is not intended
as a comprehensive discussion of the visa application process and formal legal advice
is recommended.
