J-1 Visa waiver documents

First question:
Does any one know what are the documents that I must submit to complete my waiver review application ?

I have already sent my J-1 Visa Waiver Review Application to US Department of State and now I have to wait 6 weeks to receive my case number and instruction sheet with the list of that documents, but I would like to know what documets I must to send later, so I can start prepare them.....

Second question is:
How long does it take for INS to aprove and send me final waiver decision ?.... and how long does usualy take whole proccess ?...

I already know:
1. aprox. 6 weeks to get case number and instruction sheet
2. couple of weeks to get "No Objection" statement from my government
3. I do not know how long does take me to collect all required documentation becouse i don't what i need (first question)
4. 4-8 weeks to forward by Review Division its recommendation directly to the Immigration and Naturalization Service
5. INS - How long does usualy take to get final decision ?
