instructions for filling form I-485
 

I have an approved F4 petition of October 90 and I have received an intimation from NVC asking for my choice between AOS and CP. I am currently in US on H4. I have opted for AOS. NVC has asked me to contact the local INS for getting the file tranferred from NVC to the local INS. I am filling forms for filing I-485 based on F4 category petition. I would like to know the following:
1) what is address-c/o: in Part 1.Should we write our name again as we are not living with any other family.

2) What is A# in Part 1 ? Is it the passport # or the A# mentioned on the left hand corner of I-797(approval notice of I-130).

3)Part 2. Application type: I have marked (a) in my form and (b) in my spouse's form. Am I correct?

4) Which form should I submit for affidavit of support? I am on H4 and my husband is on H1. We have maintained a lawful status for 4 years under the same category. I am told that the petitioner need not give any affidavit of support in our case since my husband is already employed. Is there any other form for affidavit of support which my husband can fill and we can attach to the application.
