Default immunization records for k1 interview

Hi all. I understand that I need my immunization records for myself and my K2 child for the interview at the London Embassy...for my fiance visa medical. I am confused as to whether they need a signed by doctor print out of all vaccinations had in the last ten years or if they send me a form to take to my doctor to get filled out. My surgery informed me that for a signed print out it would cost me £9 each...so thats £18!! I dont wanna do this and pay that if it turns out that I have to give them a form to fill in and sign. Has anyone else had this? Can anyone advise me please?

It's a good idea to have a printout for your child - you'll need that for when you get to the US (for the paediatrician and school).

Under the data protection act, they are entitled to charge you up to £10 for the information they hold on you. It is really stingy of them to do this, but I think it is worth having for the child so worth coughing up the money. You should be able to get a copy of the entire medical record for this fee.

If the information they have on you is going to prevent you from needing vaccinations for the visa, then it's worth paying too. A jab at Dr Phelan's costs around £20 - 30.

Other part of your q: there is no form for your doctor to fill in. The closest is a chart with the vaccination requirement.

What Dr Phelan needs is anything that shows whether you have had the relevent vaccinations or suffered the diseases, with a Dr or Hospital stamp and initials/signature. For my medical, I had a photocopy of medical notes (mumps), photocopy of original vaccination record (measles), photocopy of hospital blood test result (rubella) and travel clinic record (tetanus). All were acceptable.
