When I had to do this, we sent in a notarized copy of the passports with the tax return and the form to the address they asked you to send it to, somewhere in Texas from memory. They process your return and send back the ITIN numbers for you. I made the mistake of thinking i had to send off another return afterwards, but that was rejected as they processed it from the first one.
