Default DV-2004 form filling queries

Hi everyone,

I am new to this forum. I appologise in advance if the questions I am going to ask has already been answered previously.

1.
It is stated in the Intruction that I applicant have to sign his/her name in native alphabet. But my signature that I have been using for all my documents etc... is in English. So my question is how I suppose to sign the application form? Should I just go ahead and sign the signature I always use and which is also in my passport?

2.
It also stated that photo has to be attached to the form with clear tape. What does this mean? Should we stick the clear tape over the photo or put the photo into a small transparent plastic bag and then stick it to the form....or.....?

3.
If the application includes a child under 21, should the child photo be included with the application form as well? If yes, how should the child photo stick to the application form together with his/her parent's photo? Does that child has to sign the form as well?

4.
Are we allow to fill in the form electronically and then print the form out and sign it? or the form must be filled by hand? Must we fill in the form with ALL CAPITAL letters?

5.
If parents got children over 21 years old, must the children's details be included in the application form?

I hope my questions are clear.

Thank you for your time.

Alex

I would use two signatures in such a case. Sign in English, and also write
your name in your native alphabet. In fact, that's what the instructions
say.

I think almost anything of that nature would be acceptable (although I
wouldn't put it into a transparent bag). Yet another variant is to roll
the tape around your finger, sticky side out. You'd end up with a loop.
Stick that loop to the photo. Then stick the photo to the application.
Voila - you made your own double-sided tape.

You need to include a photo of all children and your spouse.

You can fill out the form on a computer and then print it. Only the
signature must be by hand. I would not use capital letters.

The instructions say only to include children under 21. See
