I am almost finished completing forms DS-230(part1), DS-156 and DS-156k, and I have a few questions that I need help with...

FORM DS-230

14. Name of Spouse
Should I write my Fiance's name?? or N/A?

15. Father's Family name?
My father or her father?

THEN...

FORM DS-156

10. NATIONAL IDENTIFICATION NUMBER
What is this - I am in the UK, never heard of it?? Could it be my National Insurance Number??

Its a biographic form about you so its your Father and Mother not the petitioner

No, it is not your NI number. It is also not for the Canadian SIN as
someone else asked recently

I have read many threads here over the past 2 years or so that ask what
this number is and have never seen an answer. I searched the web out of
curiousity and couldn't find an answer that way either. I would say
leave it blank (which is what my wife did). Perhaps N/A or even "none"
are possible responses as well.
