Unfortunately there isn't any proven medical treatment for fatigue when tests are normal. Your best hope is to exercise getting your heart rate up to 75%-85% of maximum for a minimum of 30 minutes per day for at least 5 days per week. If you can do it for 45 minutes per day or longer, that is even better and results should start occurring in about 3 weeks. That seems to be a good treatment for depressions also.

I suspect the high heart rate opens up (dilates) the blood vessels and gets more blood to the brain.

Per Wikipedia:
Ketamine (INN) is a medication used mainly for starting and maintaining anesthesia. Other uses include sedation in intensive care, as a pain killer, as treatment ofbronchospasm, as a treatment for complex regional pain syndrome and as an antidepressant. It induces a trance like state while providing pain relief, sedation, andmemory loss.[2] Heart function, breathing, and airway reflexes generally remain functional.[2]

It's been used as a street drug and 17 clinics and providers (mostly anesthesiologist) have been using it to treat depressions. It's not FDA approved for depressions so not covered by medical insurance.

It might be covered by some insurance, off label use of medications is nothing new.

One of the pills I take is off label, not approved specifically for what I take it for, but it's been effective and its completely covered as well for me.

Many mental health medications are used for off label purposes.

I agree that is true but this is a schedule III drug, the treatment is expensive, and treated in specialty clinics by doctors that aren't trained in mental illness. It's unlikely that health insurance would cover that treatment unless you were a Hollywood celebrity with a health insurance policy with all kinds off riders. Even then, treating patients with Schedule III drugs for other purposes than approved is on the fringe of illegality. Schedule IV and V drugs are not as strict but still severely controlled.

I suspect it is not one of the treatments that is required to be covered under ACA.

Physical activity - cheapest and pretty much most effective drug there is for a variety of conditions. And almost no adverse side effects.

This is a seriously scary drug to see in use.

The hospital used this on my daughter when she broke her arm, so she wouldn't feel, hear or remember them re-setting it, as her wrist was flopping by her elbow at the time.

She was in a dazed haze for quite a while and had no idea of what was going on and no memory of the few hours before and after.

I am pretty active in the summer, riding my bike nearly daily for 30-45 minutes. Other then soreness, I can't say it helps me feel better...

Winter it's hard to fit in any kind of activity between the short days, (dark when leaving for work, dark when going home) rain, cold, snow and so on, but my job is pretty active, lots of walking, but all on concrete so I end up being sore by the end of the day.

I am going to take swim lessons and try to learn to swim, since it's about as low impact activity I can think of.

I have chronic pain as well, mostly in my back, knees, and wrists, but no reason why.

Have you tried Superfeet insoles? They aren't cheap but you can buy them for athletic shoes (hiking boots, etc.) and also for dress shoes. I use them in all my hiking boots and they really help to reduce the impact not only on feet but the rest of your body too. Just a thought.
