Hi , i am wondering if i am allowed back into the usa (im a UK citizen)this year because i have read on a website that someone is meant to take your visa (i-94) upon leaving the US. Maybe im wrong and i got mistaken by what i read but i dont remember anything being taken from me by anyone and i still have part of the visa stapled in my passport from when i entered in april last year.And i don't know of any way they could know i left the USA.
I am wondering if this will affect my ability to enter th usa again which i hope to this year.
Is the i-94 meant to be still be stapled in my passport?Its not the full visa, as part of it was ripped off upon entry into the USA.
Please let me know...preferably from someone who knos 'for sure' with '100%' accuracy.....i don't want any guesses please as i don't want to make a flight and then find out they wont let me in losing my flight money.

Yes, you are supposed to give back your departure record upon leaving usa. Normally, the person at the flight check in takes it out of your passport. They seem to have forgotten it this time.
In your situation, there is no record, that you ever left the usa and this can create problems upon your next arrival.
In case, the I-94 wasn't given back upon leaving, there is a procedure, where you can send it, after you realise the mistake. And also send with it your boarding pass as supporting evidence, that you indeed left. They will then correct your records in the computer and hopefully you won't have any problems in the future.

show proof of employment in the UK since leaving the USA.
show bank records, major purchases, There are plenty of things you can proove to the USA that you left the country.

If you still have the I-94 card in your passport, as far as the US
Immigration Services is concerned you are still in the US, and you will have
major problems when trying to re-enter the US.

I had the same thing happen to me and I was able to re-enter without a
problem. Just detach the I-94 from your passport before you re-enter.
This is most probably the airline's screw up. They are supposed to
remove the I-94 form your passport. If they mention that they do not
have record of your departure then just give them all details of the
flight that you took out of the country. Of course, all of this is
assuming that you didnt overstay your previous I-94 and everything is
genuine.

departure from USA!

My suggestion is simple... speak with them... Better yet if you still have your
old boarding pass proving that you departured from US! Some personal record or
even a stamp proving that you have entered another country before the expiration
of the I-94 limit time.
If you get your pspt stamped when getting into UK, that would be a GOOD sign
that you left US, right?
Just another note.... this "stamp" proving you left US is not valid for Canada,
Mexico and the Caribean Islands!
