I was recently sent form OF-156 by the US Embassy in London to complete and return with the rest of my documents necessary when applying for my visa. Looking at the form that was mailed to me and the form on the London US Embassy web-site I noticed a number of differences.
Which form should I use and why the differences? Is one form out of date?

Most likely, one of the forms is out of date. I can't tell you which one is. I
believe the form should state the date it was issued. Look at the margins for
something in very fine print.

Also check and see if one has simply added a few questions - then use that one.

It is also possible that both forms are perfectly good. There are quite a few
versions of the OF-156 around, in several different languages and adapted for some
local requirements.

Worst case - fill out both and attach a note explaining that you didn't know which
one to use (not a great idea, but may be the best thing!)
