Default Vaccination Supplement

Hi Everyone, I need some advice!

I got married a few weeks ago on a K1 visa and I am now preparing to file for AOS.

I need a "Vaccination Supplement" I-693 signed by a civil surgeon, here in New York. Apparantly, the doctor signs a form, charges $15 and you are all set.

However, I don't have anything from my UK medical as it was sent from the Edinburgh clinic straight to to US embassy in London (since my interview was 3 days after my medical). I didn't get a copy of the medical after the interview.

So...

Shall I contact my local GP in Scotland and ask him to send my vaccination history to the civil surgeon in New York? Will this be enough?

Or contact the clinic in Edinburgh where I got my K1 medical, and see what they can do?

Or any other ideas?

My NHS Vaccination Card worked a treat. If you do not have one ask your surgery.
