John,

I picked mine up from the local police station. I live in the Metropolitan Police
area (i.e. London).

According to the reference number in the top righthand corner, the form is called
3019A. On this same, front page is the title-

Subject Access Office

Data Protection Act 1984

Request for access to informatio held on police computers

I trust this will help you. If you're in doubt, you're better off popping into your
nearest police station. Talking face to face may help the police understand exactly
what you're after.

Best of luck,

Sarah

DITTO: everytime try and fly inside the Ukraine my flight gets cancelled while I am
at the airport. Once due to heavy fog at the destination. Once they said they did not
think they had enough fuel!!!

The form I got was called COM30 but it was totally specific to the Merseyside police and so maybe every district has a different form number, though that would be confusing. I'm guessing that as long as it has a box that you tick saying you want to know your criminal conviction history then it should be ok. My form also asked me to send 2 forms of ID - photocopy of birth certificate and pasport - so there's another clue. It also had some * telling me to tick A only if I wanted this information for imigration purposes. So hopefully the form you have has similar questions.
