Travel to US with baby-- consent letter?

Travel to US with baby-- consent letter?
My baby and I are Canadian, and we'll drive south to go visit my US fiance (baby's father) for a few days in Seattle. I believe I need a letter from him to consent this particular trip, can someone help me with an example letter?
Baby's birth still not registered with US Consulate, have to wait for her father to come to Canada to do it.
We also have our I-129 (Fiancee Petition) pending so I'm aware I might not be allowed entry to US till I get my K1 Fiancee Visa.
