I need to do a sworn statement for a US visa application for an arrest in 1994 in Spain. Does the sworn statement have to be hand written or typed? Also is the "executed date" the date of arrest or the date I sign the statement. Many thanks

It can be either typed or neatly handwritten. The 'executed date' is the date the statement is signed.
