FYI for me, I went by my former husband's name as well and filled out th e paperwork
the same way you did. I put my maiden name then other names , put my x married name
that I was presently going by. My birth certifi cate is what really counted. They
want you to go by your maiden name fo r the process of INS. Although I didn't have
anything other than my bir th cert. it was fine. I had no problems. When I
remarried for the sake of INS, I signed my marriage certificate with my maiden name.
All went well and I now carry my new husband's name. Hope this lifts a bit of fe ars
for you. Best to you
