Not many options to pay for your health insurance (unless you are eligible for welfare). There's only paying for it via employment income or investment/savings income.

What else is there? How old are your parents?

​​​​​​​OK, so realistically finding work that includes health insurance is likely to be quite difficult for them, so they should plan for insurance premiums of around $1,000 each per month. Any medical services used will incur additional costs, which could be as little as around $200 for a single (in the year) visit to a doctor, up to $'000's in the even of hospitalization for a severe illness.

​​​​​​​If they become US ciitzens, which at a minimum is going to take around 6 years, they may get reduced cost insurance through Medicare, but it still won't be cheap.
