Naturalized, applying for a US passport

My questions are addressed to those who have applied for a US passport after
naturalization.

Is it possible to omit a middle name or put just an initial when filing out the
application form?

Also, if in someone's last name an apostrophe is missing in the naturalization
certificate, can an applicant type her last name WITH an apostrophe on the passpost
application form? (Or the last name must look exactly like on that naturalization
certificate?) Thanks in advance for answering my questions.
