It's a bit mushy. The rules changed a while back - they wanted to get people
on being a "good citizen" and that means not having tonnes of parking
tickets / speeding violations, etc. What happens if you didn't put them
down, though, I couldn't say. Certainly unless you didn't pay it it wouldn't
show up on a police report or anything.

I have one other question. Where on the form do you check? What
supporting documents should we send with it? I am guessing maybe
something from the court showing fine paid?

I wouldn't report a parking ticket.
First of all, that is not a sign of "bad moral character," nor is that
a crime (well, in US terminology felony, misdemeanour, violation,
etc.), much less one of "moral turpitude."
Second of all, you could have loaned your car to someone who got the
ticket; thus, while you are responsible for the fine as it is a
regulatory offence with fewer defences-at-law than crimes and
misdemeanours, you cannot, for migration purposes, be held accountable
for it.
Third of all, the Gov't, contrary to public belief, does not have a
"supercomputer" that can cross-check all your activities and pinpoint
them to you (in fact, Gov'ts work far sloppier than businesses,
organizations, even natural persons), so they almost certainly will
not catch you in a lie, but even if they do through some miracle, see
points 1 and 2 above.
Do not give some bureaucrat at the INS an excuse to deny you
citizenship because he has a hangover - to appeal their finding or
holding you would have to show they acted "arbitrarilyy and
capriciously," a tough standard to prove. It would be much easier to
appeal a mistake of fact that you did not know a parking ticket should
have been mentioned, rather than that you did in fact deserve
citizenship but were denied it unjustly by the INS who has some fairly
broad discretionary powers as an administrative agency (for example,
they can grant or not grant certain classes of visas on a whim, well
not the INS, but the DOS of course).
Having said all that, odds are 99% that whether you mention the
parking ticket or not, either way it should not be a problem; my point
is to consider the best course of action in case a problem does arise.
