What exactly is the difference? I ordered 3 BCs from the state capital's vital
statistics office to cover everything that might come along and it has my name, sex,
date and county of birth and my parents names, ages, and states of birth, but nothing
much else.
My *original* BC contains extras such as city & hospital of birth, my parents home
address, father's occupation, etc.
Would the ones I ordered from vital statistics be considered a long form?
Thanks for your time! Shonokin

They want the one with the information on your parents.
You have it, so no more worrying about which is the right one.

In the scottish system:- The short birth certificate just states Name, Sex, Date of
Birth and Place of Birth. The long also states the Parents Names, Parent's
Occupation, Parent's Address, Mother Madien Name and the Time of the Birth.
