Interim EAD card.

Interim EAD card.
Hi all.
I applied for my EAD replacement card in the normal way but immigration has taken longer than the scheduled 90 days processing time.
Now I believe I can go to the immigration office and ask for an 'interim EAD' card.
My question is...What do I need to take with me? Do I need any photos? Is there a fee. Also does anyone know if they supply you with the card there and then?

But when I was in the bcis office for finger printing, they just said to come down with passport, the letters of reciept and that was it. But looking online, they have at least gotten past the date of my reciept, so to be honest, don't know if I can be bothered to drive that far down if I could get it any time soon...oh so lazy...i'm holding off being an unemployed bum just a bit longer

2. I765 receipt
3. Other USCIS receipts, e.g., receipts for I485/I140 filing
4. Current EAD
5. Proof of residence, e.g., drivers license
6. Copy of I765 application filed with the regional office
7. Passport with I-94 form
